小児CKD-eGFR計算 - 血清クレアチニン値、血清シスタチンC値、血清β2ミクログロブリン値、24時間および2時間クレアチニンクリアランスによる推算糸球体濾過量（eGFR）を計算します．
糸球体濾過量（GFR）算出のゴールドスタンダードであるイヌリンクリアランスは，特に小児では検査手技が煩雑です．日常診療で容易に小児の腎機能を評価するためにこれまでもGFR推算式（eGFR）が使われてきました．
しかし，これまで汎用された旧Schwartz推算式は，血清クレアチニン（s-Cr）測定が現在日本で使用されている酵素法ではなくJaffé 法であるという問題があり，新Schwartz推算式は酵素法に改められましたが筋肉量の急激に増加する思春期を含めた1歳～16歳で共通の係数を使用するという問題がありました．また民族による腎機能や筋肉量の違いもあります．そこで，日本人固有の２つのeGFR作成をおこないました．
I. 日本人小児のs-Crを利用したeGFR
2歳～18歳については，男女に分けて，下記のように身長から血清クレアチニン基準値（ref Cr）を求めます．その論文は以下に示しました．
男児については身長をHt (m)とすると，
ref Cr ＝-1.259Ht5+7.815Ht4-18.57Ht3+21.39Ht2-11.71Ht+2.628
女児については身長をHt (m)とすると，
ref Cr ＝-4.536Ht5+27.16Ht4-63.47Ht3+72.43Ht2-40.06Ht+8.778
その計算をした上で，
eGFR(ml/min/1.73m2) = 110.2×(ref Cr / s-Cr) ＋2.93
によりeGFRが求められます．
残念ながら，2歳未満ではs-Crを利用したeGFRを使うことができません．
II. 日本人小児のcysCを利用したeGFR
cysCを利用したeGFRを作成した．1ヶ月～18歳について以下の式でeGFRが求められます．その論文は以下に示しました．
eGFR (mL/min/1.73 m2) = 104.1 ×1/serum cysC (mg/L) - 7.80
また，求めたGFRが正常なのか異常なのかを知るために，GFRの基準値が必要です．以前に日本人小児のs-CrやcysCの基準値を作成したときのデータを推算式に入れることでGFR基準値表を作成しました．2歳以上18歳以下はs-Crを利用したeGFRで，3ケ月以上２歳未満はcysCを利用したeGFRで計算したもので，３か月未満の基準値はありません．
腎機能が正常の50%未満ならば赤字のアラート，75%未満ならば黄字のアラートが出るようになっています．このアプリケーションが活用され，小児CKD患者が早期発見・早期治療されて，その子たちの幸せに繋がればと思います．
